Supports Coordination
Supports Coordination services are received by all individuals enrolled in the I/DD system. Supports coordinators assist individuals in developing their individual support plans, choosing providers and informal supports, and monitoring the administration of services. The Supports Coordinator will assist participants in obtaining and coordinating needed waiver and other State plan services, as well as housing, medical, social, vocational, and other community services, regardless of funding source. Supports coordination services may be provided either by a county or by an outside agency that contracts with the county to provide these services.
Supports Coordinator
Individuals registered for Mental Retardation (MR) services are assigned a Supports Coordinator. The Supports Coordinator provides the family with information and is the liaison with all services and resources available to you. The Supports Coordinator does the following to help the person and the family:
- Assesses needs
- Explains choices
- Writes an Individual Support Plan (ISP)
- Completes Waiting List registration (PUNS)
- Locates services
- Coordinates services
- Monitors services
- Advocates
- Monitors health and safety
- Calculates individual budge
- Reviews funding options/Waiver application
