ODD Registration Process
- ___Contact local MH/MR office and register for services and supports
- ___Fill out Prioritization of Urgency of Need for Services (PUNS) form with Supports Coordinator
- ___Apply for Family Driven Support Services
Waiver Registration Process
- ___Fill out Waiver Application Form
- ___Formal Assessment by Qualified Mental Retardation Professional
- ___Determination is made by County MR office regarding eligibility for waiver services and letter is mailed to you
- ___Complete an Individual Support Plan with your Supports Coordinator
- ___Budget for ISP is determined.
